Job Summary
HOME CARE PHYSICAL THERAPY ASSISTANT (PTA) provides patient care services under the supervision of a Physical Therapist, updates care plans in coordination with the therapist and physician, plans monthly re-evaluations and schedules, documents progress, and facilitates referrals and communications with the care team. Requires completion of an accredited PTA program and current state licensure; minimum one year (prefer two) in a non-home health care setting; CPR certification; ongoing continuing education; and willingness to travel. Strong knowledge of therapy concepts and ability to treat patients with physician orders; participates in patient care conferences and coordination with care coordinators.
Required Qualifications
- completion of an accredited physical therapy assistant program
- current State licensure
- minimum of one year’s therapeutic experience in a non-home health care environment; prefer two years
- CPR certified
- minimum of twelve hours of continuing education annually
