HOME CARE PHYSICAL THERAPY ASSISTANT PTA

Job Summary
Provides patient care services along with direct treatment and follow-up as assigned by the Physical Therapist; updates the care plan in coordination with the therapist and physician; plans monthly patient re-evaluations and coordinates schedules; documents patient care data; submits visit schedules to Scheduler; communicates with intercompany contacts; discusses treatment progress with patients, families, and physicians; coordinates referrals and supervisory visits; attends conferences and mandatory in-services.
Required Qualifications
- Requires completion of an accredited physical therapy assistant program and current State licensure
- CPR certified
- Minimum of twelve hours of continuing education annually
- Minimum of one year’s therapeutic experience in a non-home health care environment; prefer two years
- Strong knowledge of theory and concepts relating to the practice of physical therapy and the ability to treat patients for whom a physician’s order exists
