Job Summary
Provides in-home physical therapy services with direct treatment and follow-up. Evaluates new patients for whom there are physician orders and develops a plan of care; documents patient care data; submits daily/weekly visit schedules; communicates with internal contacts; discusses reimbursement policies with patients/families and treatment plans with the Patient Care Coordinator, physician, and patient/family; coordinates with the Patient Care Coordinator for referrals to appropriate disciplines; provides in-service education and assists with orientation and training of staff; conducts research as needed; travel and working conditions as assigned; requires a bachelor's degree in physical therapy and state licensure; ongoing education and background checks; full-time home health PT.
Required Qualifications
- Minimum bachelor’s degree in physical therapy from an accredited school of physical therapy
- current licensure from the state where therapy will be provided
- prefer a minimum of one year’s therapeutic experience in a non-home health care environment
- minimum of twelve hours of continuing education annually
