HOME CARE PHYSICAL THERAPIST PT

HOME CARE PHYSICAL THERAPIST (PT) provides patient care services in home settings with direct treatment and follow-up. Evaluates patients newly referred with physician orders and develops a plan of care appropriate to the patient’s diagnosis. Documents patient care data thoroughly and timely to reflect progress. Submits visit schedules, coordinates with the Patient Care Coordinator and intercompany contacts, discusses treatment plans and reimbursement policies with patient/family, and collaborates with the physician and care team. Coordinates referrals to appropriate disciplines; conducts in-service education and supports training of peers and students; participates in ongoing research and orientation of new employees as needed. Requires strong knowledge of physical therapy concepts, licensure, and commitment to continuing education; travel as assigned.
