Job Summary
Implement residents' personal care plans within the Fox Run campus, focusing on their strengths, preferences, and routines while providing comfort and companionship. Deliver a holistic approach addressing physical, spiritual, emotional, and clinical needs for older adults across multiple diagnostic groups, including urinary incontinence, vestibular rehab, home safety assessment, movement disorders, and low vision. Complete documentation to meet reimbursement and regulatory requirements. Work onsite with no travel involved, offering flexibility for PRN availability of 1-2 days per week. Join an interdisciplinary clinical team supported by onsite physicians and medical centers for wellness and sick care.
Required Qualifications
- Must possess a valid and current state Physical Therapy license
- Degree and training must meet current Medicare and state requirements
- The ability to complete documentation to meet reimbursement and regulatory requirements
- ideal candidate with have 1-2 days available per week (during the weekday)
- This is Home Care with NO TRAVEL involved
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups is preferred
