Job Summary
Implement residents' personal care plans within our award-winning campus, focusing on their strengths, preferences, and routines while providing comfort and companionship. Deliver a holistic approach supporting physical, spiritual, emotional, and clinical needs across specialty programs like urinary incontinence, vestibular rehab, and home safety assessments. Complete documentation to meet reimbursement and regulatory requirements. Work onsite with no travel, utilizing our interdisciplinary clinical team and onsite physicians group to ensure comprehensive care.
Required Qualifications
- Must possess a valid and current state Physical Therapy license
- Degree and training must meet current Medicare and state requirements
- The ability to complete documentation to meet reimbursement and regulatory requirements
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups
