Job Summary
Implement residents' personal care plans while focusing on their strengths, preferences, and routines to provide comfort and companionship. Deliver a holistic approach supporting physical, spiritual, emotional, and clinical needs across multiple diagnostic groups including urinary incontinence, vestibular rehab, and home safety assessment. Complete documentation to meet reimbursement and regulatory requirements. Work onsite at Fox Run Village by Erickson Senior Living with no travel involved, offering flexibility for PRN availability during weekdays.
Required Qualifications
- Must possess a valid and current state Physical Therapy license
- Degree and training must meet current Medicare and state requirements
- The ability to complete documentation to meet reimbursement and regulatory requirements
- ideal candidate with have 1-2 days available per week (during the weekday)
- This is Home Care with NO TRAVEL involved
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups is preferred
