Job Summary
Implement residents' personal care plans with a holistic approach supporting physical, spiritual, emotional, and clinical needs. Focus on residents' strengths, preferences, and routines while providing comfort, companionship, and belonging. Complete documentation to meet reimbursement and regulatory requirements. Work with the interdisciplinary clinical team to address resident needs throughout their continuum of care. Deliver therapeutic services to older adults across multiple diagnostic groups within the award-winning Fox Run campus in Novi, Michigan, with no travel required.
Required Qualifications
- Must possess a valid and current state Physical Therapy license
- Degree and training must meet current Medicare and state requirements
- The ability to complete documentation to meet reimbursement and regulatory requirements
- ideal candidate with have 1-2 days available per week (during the weekday)
- This is Home Care with NO TRAVEL involved
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups is preferred
