Job Summary
Implement residents' personal care plans within the campus-based home care setting, focusing on their strengths and preferred routines to support physical, spiritual, emotional, and clinical needs. Provide therapeutic services for older adults across multiple diagnostic groups, including urinary incontinence, vestibular rehab, home safety assessment, and movement disorders, while completing documentation to meet reimbursement and regulatory requirements. Work onsite with the interdisciplinary clinical team to ensure resident needs are addressed throughout their continuum of care.
Required Qualifications
- Valid and current state Physical Therapy license
- Degree and training meeting current Medicare and state requirements
- Ability to complete documentation to meet reimbursement and regulatory requirements
