In-home Physical Therapist role in Brooklyn, NY (per diem) providing patient-centered physical therapy services across a defined geographic area. Responsibilities include evaluating patients’ needs for physical therapy in the home, developing individualized plans of care and goals in collaboration with physicians, patients, and families; delivering customized therapy programs to restore function and promote independence; instructing family members, PTAs, home health aides, and caregivers on therapy techniques; timely and accurate documentation; and effective communication with patients, families, and the interdisciplinary team, with discharge planning. Requires NY licensure or eligibility; travel to patients’ homes within the assigned territory; options for 1099 or W-2 employment; emphasis on flexible autonomy and patient care across up to seven days per week.
