Job Summary
In-home per diem Physical Therapist role providing PT services within an assigned geographic area in New Jersey. Responsibilities include evaluating patients’ needs for physical therapy in the home, developing individualized plans of care with physicians and families, implementing customized therapy programs to restore function and promote independence, instructing family members and aides, documenting visits accurately, and coordinating discharge planning with the interdisciplinary team. Must be a licensed PT in NJ (or eligible); travel to patients’ homes within the geographic territory required. 1099 and W-2 options available; emphasis on patient care, flexibility, autonomy, and work-life balance. Preference given to primary geographic area; mentorship available for new graduates.
