Job Summary
In-home, per diem Physical Therapist position serving patients in the Long Beach area. Responsibilities include evaluating patients, developing individualized plans of care in collaboration with physicians, patients, and families; implementing customized therapy programs to restore function and promote independence; instructing family members and caregivers; providing timely documentation; coordinating discharge planning with the interdisciplinary team; and traveling to patients' homes within the geographic territory. Requires Licensed Physical Therapist in NY or eligibility; preference for prior home health experience. Flexible schedule with options for 1099 or W-2; in-person care across multiple days per week.
Required Qualifications
- Licensed Physical Therapist in the state of New York or eligible
- Physical Therapist Program Graduate
- Ability to travel to patients’ homes within the assigned geographic territory
- Home health experience preferred but not required (mentored for documentation/compliance)
- In-home care experience or interest in in-home PT practice
- In-person work arrangement in assigned geographic area
