Job Summary
The Home Care Physical Therapist provides in-home physical therapy across Brooklyn, evaluating patients, developing individualized plans of care with physicians and families, implementing customized therapy programs to restore function and promote independence, instructing caregivers, documenting visits accurately, and coordinating discharge planning with the interdisciplinary team. Responsibilities include treating patients within the designated geographic area, delivering patient-centered therapy, ensuring compliance with agency policies, and traveling to patients’ homes. Requires NY licensure (or eligibility) and the ability to travel to patients’ homes; per diem schedule with flexible days, and both 1099 and W-2 options are offered.
Required Qualifications
- Licensed Physical Therapist in the state of New York or eligible
- Home health experience preferred but not required - training provided
- Ability to travel to patients’ homes within the assigned geographic territory
