Home Care Physical Therapist- Per Diem

Provide in-home physical therapy services across designated geographic area with flexible per-visit scheduling. Responsibilities include evaluating patients’ needs in the home setting, developing individualized plans of care in collaboration with physicians, patients, and families; planning, preparing, and implementing customized therapy programs to restore function and promote independence; instructing family members and caregivers on therapy techniques; delivering timely documentation; communicating with patients, families, and the interdisciplinary team around discharge planning; complying with agency policies and professional practice standards; travel to patients’ homes within the assigned geographic territory. Requires Physical Therapist Program Graduate and Licensed Physical Therapist in New York (or eligible); home-health experience preferred but not required, with mentor support provided. In-person remote option; per diem compensation.
