Job Summary
Per Diem Physical Therapist providing in-home care in the Farmingville, NY area. Responsibilities include evaluating patients’ needs for physical therapy in the home setting, developing individualized plans of care in consultation with physicians, patients, and families, implementing customized therapy programs to restore function and promote independence, instructing family members and caregivers, delivering timely documentation, and communicating with patients and the interdisciplinary team around discharge planning. Requires NY licensure or eligibility; flexible schedule with travel to patients’ homes within the assigned geographic area. Options for 1099 and W-2 employment; per-visit compensation of $70-$90; focus on patient care with EMR documentation and adherence to agency policies.
Required Qualifications
- Licensed Physical Therapist in the state of New York or eligible.
- Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
