Job Summary
Home-based Physical Therapist for per diem visits in Nassau County, NY. Independently evaluate patients in their homes, develop and implement individualized plans of care in collaboration with physicians, patients, and families. Travel to patients’ homes within the assigned geographic area to deliver physical therapy, instruct family members and caregivers on therapy techniques, and document care per agency standards. Flexible scheduling with options for 1099 or W-2 employment; ability to build a caseload aligned to your preferences; mentoring available for new graduates. Must be a licensed NY Physical Therapist or eligible; degree typically Doctor of Physical Therapy. Focus on restoring function and promoting independence through customized therapy programs and effective discharge planning; strong communication with patients, families, and interdisciplinary team.
Required Qualifications
