Job Summary
Per diem in-home Physical Therapist needed to evaluate patients, develop individualized plans of care, and provide therapy in the home within the Commack area. Responsibilities include planning and implementing customized therapy programs to restore function and promote independence, instructing family members and caregivers on therapy techniques, providing timely documentation, and collaborating with physicians, patients, and the interdisciplinary team around discharge planning. Travel to patients' homes within the assigned geographic territory is required. Licensed Physical Therapist in NY or eligible; home health experience preferred but not required with mentorship provided. Flexible schedule with opportunities to see patients up to seven days a week; 1099 and W-2 options available.
