Job Summary
Provide in-home physical therapy within Nassau County, NY, evaluating patients, developing and implementing plans of care, and delivering customized therapy programs in the home setting. Instruct family members and caregivers on therapy techniques, document per visit, coordinate discharge planning with physicians and families, and travel to patients' homes within the designated geographic area. Requires NY licensure or eligibility as a Physical Therapist; home-health experience preferred but not required and mentoring will be provided. Flexible schedule with 7 days of patient access; options for 1099 or W-2 employment; focus on autonomy, patient independence, and high-quality care.
Required Qualifications
- Licensed Physical Therapist in the state of New York or eligible
- Ability to travel to patients’ homes within the assigned geographic territory
