Job Summary
Per diem Home Care Physical Therapist to provide in-home physical therapy across Holtsville area. Responsibilities include evaluating patients, developing individualized plans of care with physicians and families, implementing customized therapy programs to restore function and promote independence, instructing family members and caregivers, providing timely documentation, and collaborating with an interdisciplinary team. Must have NY license (or be eligible); travel to patients’ homes within the assigned geographic territory; flexible schedule; 1099 and W-2 options available; focus on patient care and autonomy in home settings.
Required Qualifications
- Licensed Physical Therapist in NY or eligible
- Physical Therapist Program Graduate
