In-home Physical Therapist to provide per diem PT services in the North Shore/Nassau County area. Responsibilities include evaluating patients, developing and implementing individualized home therapy plans, instructing families and caregivers on therapy techniques, ensuring timely documentation, and collaborating with physicians and the interdisciplinary team. Flexible schedule with the option for 1099 or W-2; travel to patient homes within the assigned geographic area; emphasis on patient care, discharge planning, and adherence to professional practice standards; focus on restoring function and promoting independence.
Required Qualifications
- Licensed Physical Therapist in NY or eligible
