Job Summary
Per Diem Home Care Physical Therapist in Manhattan conducting in-home physical therapy visits. Responsibilities include evaluating patients in their home, developing individualized plans of care in consultation with physicians, patients, and families, implementing customized therapy programs to restore function, instructing family members and caregivers on therapy techniques, providing documentation for each visit, and collaborating with the interdisciplinary team around discharge planning. Travel to patients' homes within the assigned geographic area is required. NY licensure or eligibility is required; flexible schedule and mentor support available; 1099 and W-2 options offered. Salary is paid per visit ($75-$90) with pay based on experience and geography.
Required Qualifications
- Licensed Physical Therapist in the state of New York or eligible
- Home health experience preferred but not required
