Job Summary
Provide in-home physical therapy as a per diem clinician within the designated geographic area. Evaluate patients’ needs for physical therapy in the home, develop individualized plans of care and treatment goals in consultation with physicians, patients, and families, and implement customized therapy programs to restore function and promote independence. Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques; deliver timely documentation; communicate with patients, families, and the interdisciplinary team, especially around discharge planning; and travel to patients’ homes within the assigned territory. 1099 and W-2 options are available. Licensed Physical Therapist in NY or eligible is required; home health experience is preferred but not required. Flexible schedule with care delivered across 7 days per week.
Required Qualifications
- Licensed Physical Therapist in the state of New York or eligible
- Physical Therapist Program Graduate
