Job Summary
Per diem Home Care Physical Therapist in Brooklyn, NY. Responsibilities include evaluating patients within your designated geographic area, developing plans of care in consultation with physicians, patients, and families, planning, preparing and implementing customized therapy programs to restore function and promote independence, instructing family members, PTAs, Home Health Aides, and caregivers on therapy techniques, providing timely documentation, and coordinating discharge planning. Travel to patients’ homes is required within the assigned geographic area. Licensed Physical Therapist in NY or eligible; Certifications: First Aid, AED, CPR. Flexible schedule with the option to see patients on up to 7 days per week; 1099 and W-2 options available; pay per visit between $75-$90.
Required Qualifications
- Licensed Physical Therapist in the state of New York or eligible
- Physical Therapist Program Graduate
- Mandatory Certifications: First Aid, AED, & CPR
