Job Summary
Part-time Home Care Physical Therapist in Manhattan, NY, providing in-home physical therapy within the designated geographic area. Responsibilities include evaluating patients, developing individualized plans of care in collaboration with physicians, families, and the interdisciplinary team; planning and implementing customized therapy programs to restore function and promote independence; instructing family members and caregivers on therapy techniques; timely documentation; coordinating discharge planning; and traveling to patients’ homes. Licensed NY PT or eligible required; home health experience preferred but not required, with mentorship available for new graduates. Flexible schedule with autonomy to see patients across up to seven days per week. Competitive salary with comprehensive benefits.
