Job Summary
Provide in-home physical therapy in the designated geographic area, evaluating patient needs, developing individualized plans of care in collaboration with physicians, patients, and families; deliver customized therapy programs to restore function and promote independence; instruct family members and caregivers on therapy techniques; ensure timely, accurate documentation; communicate with patients, families, and the interdisciplinary team, and travel to patients' homes within the assigned territory. Licensed NY PT or eligible required; part-time with benefits; flexible schedule including up to 7 days/week; travel within geographic area; incentives and professional development opportunities.
Required Qualifications
- Licensed Physical Therapist in the state of New York or eligible
- Home health experience preferred but not required - mentorship provided for documentation and compliance
