Job Summary
Provides patient care services with direct treatment and follow-up in the home-health setting; evaluates patients new to service with physician orders and develops a plan of care appropriate to the patient’s diagnosis; documents patient care data accurately and timely; submits daily/weekly visit schedules to the Scheduler; communicates with internal contacts; discusses reimbursement policies with patient/family and treatment plans with care team; coordinates referrals to appropriate disciplines; provides in-service education and supervises peers and students; conducts research and assists with orientation of new employees; requires a minimum Bachelor’s degree in Physical Therapy and current state licensure; continuing education annually; travel as assigned.
Required Qualifications
- Bachelor’s degree in Physical Therapy from an accredited school
- Current licensure from the state where therapy will be provided
- Prefer a minimum of one year’s therapeutic experience in a non-home health care environment
- Minimum of twelve hours of continuing education annually
- Accept travel and working conditions as assigned
