Job Summary
Provide home care physical therapy services, evaluate patients with physician orders, and develop a plan of care appropriate to the patient’s diagnosis. Document patient care data accurately and thoroughly, submit visit schedules, and coordinate with the Patient Care Coordinator and other team members. Discuss treatment plans with patients, families, and clinicians; provide in-service and public education; supervise peers and students as needed; participate in orientation and training; and travel as required to deliver care in home settings.
Required Qualifications
- Minimum bachelor’s degree in physical therapy from an accredited school of physical therapy and current licensure from the state where therapy will be provided.
- Prefer a minimum of one year’s therapeutic experience in a non-home health care environment.
- Minimum of twelve hours of continuing education annually.
- Accept travel and working conditions as assigned.
