Job Summary
Accept and treat patients within your designated geographic area, providing 1:1 treatment and delivering care aligned with professional standards and the prescribed plan of care. Evaluate patients to develop individualized treatment plans based on physician orders, perform initial exams to determine diagnoses, and adjust treatments to achieve maximum benefit. Educate family members and caregivers on proper therapy techniques while providing timely documentation for each visit. Must be willing to treat geriatric patients and able to travel to homes within your assigned territory. Flexible schedule allows seeing patients any 7 days of the week, with preference given to your primary geographic area.
Required Qualifications
- Licensed Physical Therapist in the state of New Jersey or eligible
- Must be willing to treat geriatric patients
- Ability to travel to patients' homes within the assigned geographic territory
Desired Qualifications
- Physical Therapist Program Graduate
- First Aid, AED, & CPR Certifications
- Home health experience
