Job Summary
Provide in-home physical therapy in the designated geographic area of New Jersey, evaluating patients and developing individualized plans of care in collaboration with physicians, patients, and families. Deliver customized therapy programs to restore function and promote independence, instruct family members and caregivers, and maintain timely documentation. Licensed Physical Therapist in New Jersey or eligible required; new graduates welcomed with training. Must be able to travel to patients’ homes within the territory and coordinate with interdisciplinary team around discharge planning; EMR documentation and compliance with agency policies.
