Job Summary
In-home physical therapy role delivering patient-centered care within the designated geographic area. Responsibilities include evaluating patients’ needs, developing and implementing individualized treatment plans, planning and implementing plans of care in consultation with physicians, patients, and families; guiding and educating family members and caregivers on therapy techniques; ensuring timely, accurate documentation; communicating with patients, families, and the interdisciplinary team, especially around discharge planning; delivering care in accordance with agency policies and standards; travel to patients’ homes within the assigned territory.
Required Qualifications
- Licensed Physical Therapist in the state of New York or eligible
- Physical Therapist Program Graduate
