Job Summary
In-home Physical Therapist position in Patchogue, NY. Responsibilities include evaluating patients’ needs in their home setting, developing individualized plans of care in consultation with physicians, patients, and families, implementing customized therapy programs to restore function and promote independence, instructing family members and caregivers on therapy techniques, providing timely documentation, and coordinating discharge planning with an interdisciplinary team. Requires NY licensure or eligibility; travel to patients’ homes within the geographic area; flexible schedule with care across up to seven days. Required Qualifications
- Licensed Physical Therapist in the state of New York or eligible
