Job Summary
In-home Physical Therapist needed in Center Moriches, NY to evaluate and treat patients in their homes within the designated geographic area. Develop individualized plans of care in consultation with physicians, patients, and families; implement customized therapy programs to restore function and promote independence; instruct family members and caregivers on therapy techniques; provide timely documentation; communicate with patients, families, and interdisciplinary team members around discharge planning; travel to patients’ homes as needed. Requires NY licensure or eligibility; new graduates welcomed and mentored. Competitive salary ($90k–$100k), benefits, PTO, and growth opportunities. Required Qualifications
- Licensed Physical Therapist in the state of New York or eligible
- Home health experience preferred but not required
- Physical Therapist Program Graduate
