Job Summary
Provide in-home physical therapy within the designated geographic area, evaluate patients’ needs, develop and implement individualized plans of care in collaboration with physicians, patients, and families, and guide caregivers and staff on therapy techniques. Deliver timely documentation, communicate with patients, families, and the interdisciplinary team (including discharge planning), travel to patients’ homes within the assigned territory, and maintain professional practice standards. Required Qualifications
- Licensed Physical Therapist in the state of New York or eligible
- Physical Therapist Program Graduate
