Job Summary
Conduct thorough evaluations of patients recovering from injuries, surgeries, or chronic health issues within their home environment. Develop and implement individualized treatment plans utilizing therapeutic exercises, manual therapy, and assistive device training. Continuously monitor and document patient progress, adjusting strategies as necessary to achieve optimal outcomes. Educate patients, families, and caregivers on effective self-care practices. Collaborate with interdisciplinary teams to coordinate care and attend team meetings when necessary. Stay current with best practices through professional workshops and local PT associations. This onsite role serves children to seniors with unique rehabilitation needs, emphasizing adaptability to varied home settings. Flexible scheduling primarily during weekdays supports work-life balance.
Required Qualifications
- A Doctorate degree in Physical Therapy (DPT) from an accredited program.
- A valid state license to practice Physical Therapy in Florida.
- Excellent verbal and written communication skills to effectively interact with patients, families, and healthcare team members.
- Basic proficiency in electronic health records and other clinical documentation systems.
- A flexible and proactive mindset, with the ability to adapt to different patient needs and situations.
Desired Qualifications
- At least two years of relevant clinical experience in a physical therapy setting.
- Experience in home health care.
