Job Summary
Conduct thorough patient evaluations, develop individualized treatment plans, and monitor progress for clients recovering from surgeries, injuries, or managing chronic conditions. Engage patients and families in care goals while documenting assessments, daily notes, and discharge plans meticulously. Manage schedules efficiently to meet productivity expectations and collaborate with other healthcare professionals on interdisciplinary treatment approaches. Participate in departmental meetings, contribute to continuous improvement initiatives, and engage in community outreach to promote services.
Required Qualifications
- A Bachelor's Degree in Physical Therapy from an accredited institution
- A valid Physical Therapist license in the state of New Jersey
- Current BLS CPR certification from the American Heart Association
- Strong interpersonal and communication skills to facilitate effective interactions with patients and their families
Desired Qualifications
- A minimum of two years of experience in a physical therapy setting, preferably in home care or outpatient environments
