Job Summary
Conduct comprehensive evaluations of patients in home settings, assessing physical capabilities and determining treatment needs. Develop and implement detailed, individualized treatment plans utilizing modalities such as manual therapy, therapeutic exercises, and functional training. Monitor and document patient progress meticulously, maintaining accurate records to track effectiveness and refine goals. Utilize patient feedback to adjust treatment plans dynamically, educate patients and families on self-management techniques, and train caregivers in assistive device application. This role involves traveling to patients' homes within the Fords, New Jersey area, with a flexible Monday to Friday schedule.
Required Qualifications
- Bachelor's Degree in Physical Therapy from an accredited institution
- Must hold a valid state license to practice as a Physical Therapist
- Excellent verbal and written communication skills to effectively interact with patients, families, and care teams
- Proficiency in computer software and applications relevant to physical therapy documentation and patient management
- Must be able to lift 50 lbs
