Job Summary
Conduct thorough patient assessments to develop personalized care plans and implement therapeutic interventions based on physician-established outcomes. Provide direct physical therapy services within the patient's home, utilizing evidence-based methodologies to promote functional recovery for diverse populations including post-surgical and chronic pain cases. Educate patients and families on therapeutic exercises and adaptive device usage while monitoring progress and modifying treatment plans as necessary. Maintain clear documentation in compliance with legal standards and prioritize caseload coordination with the healthcare team. Requires a valid California DPT license, BCLS certification within 30 days, and a salary range of $100,000 to $120,000.
Required Qualifications
- A valid license to practice physical therapy in the state of California
- A Doctor of Physical Therapy (DPT) degree
- Basic Cardiac Life Support (BCLS) certification
