Job Summary
Conduct comprehensive evaluations of patients referred by physicians to establish baseline data and needs. Develop, implement, and adjust individualized treatment programs using manual therapy, therapeutic exercise, and modalities. Provide education and training to patients and caregivers regarding home modifications and exercise routines. Maintain thorough documentation and progress notes while collaborating with a multidisciplinary team to optimize care. Work Monday through Friday in a standard 40-hour week, delivering personalized care directly in patients' homes.
Required Qualifications
- A Bachelor's Degree in Physical Therapy from an accredited program
- A valid license to practice Physical Therapy in the state of Virginia
- Completion of Basic Life Support (BLS) certification from the American Heart Association
- Strong written and verbal communication skills
- Proficiency in word processing and electronic documentation systems
- A demonstrated ability to connect and build relationships with patients
Desired Qualifications
- A Doctorate Degree in Physical Therapy (DPT)
- At least 2 years of relevant experience in a clinical setting
- Experience in home health care
- Experience in outpatient environments
- Experience treating a range of conditions across different age groups
- A history of using evidence-based practice to achieve successful patient outcomes
