Job Summary
Conduct comprehensive patient evaluations in home environments to develop individualized treatment plans. Utilize therapeutic modalities including exercise programs and manual therapy to alleviate pain and improve mobility. Continuously assess progress, adjust strategies, and provide education to patients and families on proper techniques and assistive device use. Collaborate with interdisciplinary teams during meetings to ensure continuity of care. Maintain current industry knowledge through workshops and publications. Work Monday through Friday with flexible scheduling options.
Required Qualifications
- A Doctorate degree in Physical Therapy (DPT)
- Active State License in Physical Therapy
- Strong verbal and written communication skills
Desired Qualifications
- Proficiency in word processing
- experience with electronic health records (EHR) systems
- Flexibility and adaptability in a dynamic healthcare environment
- At least two years of clinical experience as a Physical Therapist in a home care or related setting
- Previous experience with treatment planning and execution for a variety of conditions
