Job Summary
Conduct thorough evaluations of patients' physical abilities and limitations in their home environments, then develop and implement personalized treatment plans based on assessment results. Utilize therapeutic techniques including exercise programs, manual therapy, and education on managing conditions effectively. Maintain detailed documentation of patient progress and update treatment plans as necessary to reflect evolving needs. Educate patients and families on exercises, proper use of assistive devices, and strategies for enhancing mobility and function. Collaborate with multidisciplinary teams, including caregivers and other healthcare professionals, to ensure comprehensive patient care. Stay current with best practices by attending workshops, conferences, and engaging with professional organizations.
Required Qualifications
- Current state licensure or eligibility to obtain state licensure in Physical Therapy
- Strong verbal and written communication skills to effectively engage with patients, families, and team members
- Proficient in digital documentation and health care systems
- Flexibility and adaptability in a dynamic home care environment
- A commitment to ongoing learning and professional development
Desired Qualifications
- Doctorate in Physical Therapy (DPT)
