Home Care Physical Therapist- Full Time (North Shore, Nassau County)

Provide in-home physical therapy to patients within the designated geographic area. Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans; plan, prepare, and implement customized therapy programs to restore function and promote independence; instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques; deliver timely and accurate documentation for each visit; communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning; deliver care in compliance with agency policies and professional practice standards; Licensed Physical Therapist in NY or eligible, with ability to travel to patients’ homes.
