Job Summary
Provide physical therapy services including assessments, treatment planning, and therapeutic interventions within professional practices and ethical standards. Administer therapy statements, evaluate treatment effects, and document patient care services for billing purposes. Help patients accomplish treatment goals by securing adaptive equipment and instructing families in home exercise programs. Supervise physical therapy staff, complete discharge planning, and ensure compliance with CMS, federal, and state regulations. Maintain professional licensure and technical knowledge while following safety policies and regulations. Full-time M-F 8am-4:30pm schedule with 80 hours bi-weekly.
Required Qualifications
- Bachelor's degree in Physical Therapy
- Previous experience working as a Physical Therapist
- Current license, state certification, or registered as a Physical Therapist in good standing
- Valid Drivers License
- proof of insurance

- Ability to communicate effectively in both verbal and written formats
- Must be flexible and adaptable to changing situations
- able to work irregular hours
- attend meetings and special events
- Ability to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident's Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to know, SDS, ADA, HUD Policy and Procedures, Fair Housing laws and regulations

Desired Qualifications
- Doctor of Physical Therapy (DPT) degree
- 2+ years previous experience in home health, hospice or senior care
