Home Care Physical Therapist- Full Time (Eastern Suffolk)

Accept and treat patients within your designated geographic area, evaluating needs for in-home physical therapy and developing individualized treatment plans. Create and implement customized therapy programs to restore function while instructing family members and caregivers on appropriate techniques. Provide timely documentation for each visit and communicate effectively with patients, families, and the interdisciplinary team regarding discharge planning. Deliver care in compliance with agency policies and professional practice standards. This role offers flexibility to see patients any seven days of the week with a preference for your primary geographic area.
