Job Summary
Conduct comprehensive patient evaluations and develop personalized treatment plans for home care settings. Implement therapeutic interventions across orthopedic, pediatric, neurological, and sports medicine specialties while delivering skilled services in patients' homes. Educate patients and caregivers on recovery techniques and collaborate with multidisciplinary teams to optimize outcomes. Maintain physician-approved protocols to promote healing and functionality while achieving established performance goals.
Required Qualifications
- Graduation from an APTA or CAPTE-approved physical therapy program
- Doctorate degree in Physical Therapy
- A current and valid State License in Physical Therapy
- Basic Life Support Certification from the American Heart Association
- Excellent verbal and written communication skills
- Proficiency in word processing and various computer applications conducive to managing patient records and reports
- Ability to establish strong connections with patients, aiding them in reaching and surpassing their functional goals
- A passion for building relationships within the community and connecting positively with individuals from various backgrounds
Desired Qualifications
- Experience in home health care or working within a community-based therapy setting
