Job Summary
Provide skilled home health physical therapy services under the direction of a licensed Physical Therapist, implementing individualized treatment plans to improve mobility, balance, strength, and functional independence for patients recovering from illness, injury, surgery, or neurological disorders. Perform home visits delivering compassionate, patient-centered care while observing progress, conducting assessments, documenting findings, and communicating changes to the supervising provider. Modify interventions as directed to help patients achieve established goals, educate caregivers on home exercise programs and safety techniques, and collaborate with the interdisciplinary team. Maintain accurate documentation ensuring compliance with Medicare guidelines and Virginia regulations.
Required Qualifications
- Active Virginia Physical Therapist Assistant (PTA) License
- Graduate of an accredited Physical Therapist Assistant program
- Minimum of one (1) year of clinical experience as a Physical Therapist Assistant, preferably in home health or another post-acute care setting
- Knowledge of state and federal home health regulations
- Strong clinical judgment, communication, interpersonal, and documentation skills
- Current CPR certification (or ability to obtain)
- Valid driver's license, reliable transportation, and proof of auto insurance
