Job Summary
Conduct in-home assessments to evaluate clients' daily living needs and challenges. Create and carry out personalized treatment plans tailored to patient requirements. Administer therapeutic exercises, manual therapy, and other interventions to enhance functional abilities. Collaborate with clients, families, and healthcare professionals to ensure coordinated care. Educate clients on proper techniques for home exercises and self-care. Maintain accurate documentation of assessments, interventions, and progress. Required Qualifications
- Degree in Physical Therapy from an accredited program
- State licensure as a Physical Therapist Assistant
