Job Summary
In-home physical therapy for patients within the designated geographic area in South Shore/Nassau County. Provide evaluation, develop and implement a plan of care and goals in collaboration with physicians, patients, and families. Deliver customized therapy programs to restore function and promote independence; educate family members and caregivers on treatment techniques; document visits accurately; coordinate discharge planning with the interdisciplinary team. Licensed NY PT or eligible required; travel to patients’ homes within territory; flexible schedule with opportunities for both 1099 and W-2 arrangements; in-person visits only.
Required Qualifications
- Licensed Physical Therapist in the state of New York or eligible
- Per Diem employment
- Ability to travel to patients’ homes within the assigned geographic territory
- Home health experience preferred but not required (training provided)
