Home Care Physical Therapist-1099 (North Shore, Nassau County)

Job Summary
Provide in-home physical therapy to patients within your designated geographic area; evaluate patients’ needs for physical therapy in the home setting and develop individualized plans of care in consultation with physicians, patients, and families; plan, prepare and implement customized therapy programs to restore function and promote independence; instruct family members, PTAs, Home Health Aides, and caregivers on therapy techniques; deliver timely and accurate documentation for each visit; communicate effectively with patients, families, and interdisciplinary team members around discharge planning; ensure care complies with agency policies and professional standards; NY Licensed Physical Therapist or eligible; travel to patients’ homes; flexible scheduling; 1099 and W-2 options available; preference given to see patients in the primary geographic area; per diem payments of $70-$90 per visit.
Required Qualifications
- Licensed Physical Therapist in the state of New York or eligible
- Home health experience preferred but not required
- Ability to travel to patients’ homes within the assigned geographic territory
