Job Summary
In-home Physical Therapy role in Manhattan as a per diem contractor providing evaluation and individualized plans of care for patients in their homes. Responsibilities include evaluating patient needs, developing and implementing customized therapy programs, instructing family members and aides, delivering timely documentation, and coordinating discharge planning with physicians and the interdisciplinary team. Requires NY state licensure (or eligibility) and certifications in First Aid, AED, and CPR; travel to patients’ homes within the designated geographic area; option for 1099 and/or W-2 employment; flexible schedule and autonomy in choosing patients within the primary geographic area.
Required Qualifications
- Licensed Physical Therapist in the state of New York or eligible
- Mandatory Certifications: First Aid, AED, & CPR Certifications
