Home Care Physical Therapist-1099 (Eastern Suffolk County)

Accept and treat patients within your designated geographic area in the home setting. Evaluate patient needs and develop individualized treatment plans, then create and implement customized therapy programs to restore function and promote independence. Instruct family members, PTAs, and caregivers on appropriate techniques while providing timely documentation for each visit. Communicate effectively with patients, families, and the interdisciplinary team regarding discharge planning, ensuring all care complies with agency policies and professional standards. Deliver care with flexibility to see patients any day of the week, with preference for your primary area. Licensed Physical Therapist in New York required; home health experience preferred.
